HCPCS Modifier XU
Unusual Non-Overlapping Service

What modifier XU means
Modifier XU is one of the four X{EPSU} subsets of modifier 59, indicating a service is distinct because it did not overlap the usual components of the main service. It gives a more specific reason than 59 for unbundling an NCCI procedure-to-procedure edit pair.
When to use it
Two edit-paired services are reported together and the second is separate because it does not overlap the usual components of the primary service.
Documentation checklist
The record should support every item below before you append modifier XU.
- How the service was non-overlapping with the primary procedure's usual components
- The separate work performed
- Independent medical necessity for the additional service
Do NOT use modifier XU when
- A more specific subset applies — separate encounter (XE), structure (XS), or practitioner (XP)
- The services are genuinely bundled and not separable
- Anatomic laterality (LT/RT) better describes the distinction
Common denial reasons
- The NCCI edit is not bypassable, or the documentation does not establish non-overlap
- Payer does not recognize the X{EPSU} modifiers and still expects 59
- Overuse drawing scrutiny
How to appeal a modifier XU denial
Attach the documentation showing the second service did not overlap the primary procedure's usual components and cite the specific NCCI edit being bypassed. If the payer rejected X{EPSU}, resubmit with modifier 59 per that payer's policy.
Payer notes
CMS prefers the specific X{EPSU} modifiers over 59 when one applies. Some commercial payers still accept only modifier 59 — check payer policy before substituting XU.
