What modifier XP means
Modifier XP is one of the X{EPSU} subsets of modifier 59, indicating a service is distinct because it was performed by a different practitioner. It is the most specific reason when the distinctness between two edit-paired services is provider identity.
When to use it
Two edit-paired services are performed by different practitioners — for example, one clinician performs a procedure while a different clinician furnishes a separate, distinct service.
Documentation checklist
The record should support every item below before you append modifier XP.
- The separate practitioners involved
- Each provider's distinct role in the services
- Independent medical necessity for each service
Do NOT use modifier XP when
- The real distinction is a separate encounter (XE), structure (XS), or non-overlap (XU)
- The same practitioner performed both services
- The pair is not separable
Common denial reasons
- The separate-practitioner distinction is not documented
- Payer expects modifier 59 rather than X{EPSU}
- The NCCI edit is not bypassable
How to appeal a modifier XP denial
Document the two distinct rendering practitioners and each one's separate, medically necessary service, and cite the NCCI edit being unbundled. If XP was rejected, resubmit with modifier 59 per the payer's policy.
Payer notes
CMS prefers XP over modifier 59 when the distinctness is the practitioner. Some commercial payers still require 59 — confirm payer policy before substituting XP.
