What modifier XS means
Modifier XS is one of the X{EPSU} subsets of modifier 59, indicating a service is distinct because it was performed on a separate organ or anatomic structure. It is more specific than 59 when the reason two edit-paired procedures are separable is anatomic.
When to use it
Two edit-paired procedures are performed on different organs or structures — for example, lesions excised at separate, distinct anatomic sites.

The record should support every item below before you append modifier XS.
- The separate organs or structures involved
- Site specificity for each procedure
- Independent medical necessity for each
Do NOT use modifier XS when
- The real distinction is a separate encounter (XE), practitioner (XP), or non-overlap (XU)
- The procedures were on the same structure
- The services are not actually separable
Common denial reasons
- Separate structures not clearly documented
- Payer does not accept X{EPSU} modifiers and expects 59
- The NCCI edit is not bypassable
How to appeal a modifier XS denial
Submit documentation naming the separate organs or anatomic structures and cite the NCCI edit being unbundled. If XS was rejected, resubmit with modifier 59 per the payer's policy, keeping the same supporting note.
Payer notes
CMS prefers XS over modifier 59 when the distinction is anatomic. Some commercial payers still require 59 — confirm payer policy before substituting XS.
