Zero Drug Amount Discarded/Not Administered to Any Patient

What modifier JZ means
Modifier JZ attests that there was no discarded amount of a single-dose-container drug — the entire vial or package was administered to the patient. It is the counterpart to modifier JW (which reports discarded drug) and, under CMS reporting policy, is required on single-dose-container drug claims to affirmatively confirm zero wastage.
When to use it
A drug supplied in a single-dose vial or package was fully administered with nothing discarded.
Documentation checklist
The record should support every item below before you append modifier JZ.
- Drug name, NDC, and dose administered
- That the drug came from a single-dose container
- That no amount was discarded
Do NOT use modifier JZ when
- Any amount was discarded — use modifier JW for the wasted units
- The drug came from a multi-use (multi-dose) vial
- The product is not a single-dose-container drug
Common denial reasons
- JZ omitted on a single-dose-drug claim where CMS edits require JW or JZ
- JZ used when waste actually occurred (should have been JW)
- NDC/units inconsistencies
How to appeal a modifier JZ denial
If a single-dose-drug line rejects for a missing wastage modifier, resubmit with JZ when nothing was discarded (or JW with a separate wasted-units line when it was), ensuring administered units and the drug's NDC are consistent with the vial size.
Payer notes
Under CMS's discarded-drug policy, single-dose-container drug claims must carry either JW (when drug is discarded) or JZ (when none is). Commercial adoption of JZ varies — confirm the payer's requirement.
