Where covered: BMI threshold is plan-dependent (BMI >=30, or BMI >=27 with >=1 weight-related comorbidity such as hypertension/T2DM/dyslipidemia on the common 4774-C design; or BMI >=35 with no comorbidity alternative on the stricter 6450-C 'BMI 35' design) plus participation in a >=6-month comprehensive weight-management program (behavior modification, reduced-calorie diet, increased activity).

Pediatric (12-17): Saxenda BMI >=30 adult-equivalent + weight >60 kg; Wegovy BMI >=95th percentile.

Initial approval Wegovy 7mo / Zepbound 8mo / Saxenda 4mo; continuation 12mo requires stable dose >=3mo and >=5% weight loss (Saxenda >=16 weeks + >=4%).

Since 7/1/2025 Wegovy is CVS Caremark's preferred anti-obesity GLP-1 and Zepbound was dropped from the standard preferred commercial formulary (does not affect diabetes form Mounjaro).
