Approval requires ONLY a diagnosis of Type 2 diabetes mellitus (documentation required); for brand Byetta/Victoza only, also a preferred-product step (tried bioequivalent generic).
NO metformin step therapy and NO A1C threshold.
Approve for 1 year.
Not covered: weight loss without T2DM, Type 1 diabetes, prediabetes/prevention, metabolic syndrome, concurrent use of two GLP-1/GLP-1-GIP agonists.
